Surgery · Performed by a general surgeon

Blepharoplasty

Surgery to remove the excess skin that gathers on the upper or lower eyelid. Done under local anaesthetic, in the clinic, with the field of vision often mattering more than the appearance.

Procedure

1–2hours

Depending on upper, lower or both.

Anaesthetic

1local

Awake, and numb.

Stitches out

5–7days

Eyelid skin heals fast.

Visible bruising

10–14days

Plan around it.

Scar settles

6–12months

It sits in the crease.

A woman around sixty looks up with open, bright eyes and a crisp lid crease.

What changes

Two weeks of looking like it. Then years of not.

This is the one treatment on this list with a recovery worth planning your calendar around.

  1. 01

Swollen and bruised for about two weeks.

  2. 02

    Stitches out within a week.

  3. 03

By a month, it reads as rested.

  4. 04

The scar sits in the crease and fades.

How it works

Skin is removed. The crease hides the scar.

Eyelid skin is the thinnest on the body, and it stretches. Over years the upper lid accumulates more skin than the socket can hold, so it folds — first into the crease, eventually onto the lashes, and in some people far enough to narrow the upper field of vision.

The operation removes a measured ellipse of that excess and closes the incision inside the natural crease, where the resulting scar sits in a fold and becomes very hard to see. On the lower lid the concern is usually fat rather than skin, and the approach differs accordingly. Which of those you have, and whether the real problem is the brow above rather than the lid itself, is what the assessment establishes.

Close view of a smooth upper eyelid and its natural crease.
  1. 01

    The skin stretches

Eyelid skin is the thinnest on the body and accumulates faster than the socket can hold it.

  2. 02

    The excess is measured

Marked with you sitting upright and your eyes open, because that is how the lid actually sits.

  3. 03

    The crease takes the scar

The incision closes inside the natural fold, where it becomes very hard to see.

Your visit

An assessment first. Then the procedure.

Unlike the injectables, this is two separate appointments, and deliberately so.

  1. 01

    First appointment

    The assessment

Photographs, a measurement of the excess with you sitting upright, and a discussion of whether the lid or the brow is the actual problem.

  2. 02

    Before you commit

    The plan and the price

What will be removed, what the recovery looks like week by week, and the figure, agreed in writing.

  3. 03

    Second appointment

    The procedure

One to two hours under local anaesthetic. You are awake, numb, and go home the same day.

  4. 04

    5 to 7 days later

    Stitches out

A short visit. Eyelid skin heals faster than skin elsewhere.

Preparation and aftercare

Before and after your procedure.

  • List your medications

Blood thinners especially. Do not stop them yourself.

  • Arrange a ride home

You will be dressed and not driving.

  • Clear two weeks in the calendar

Bruising is visible for about that long.

  • Stop smoking if you can

    It measurably slows wound healing.
